Matrix is PCHI’s electronic patient registry system for managing/coordinating/tracking patient care. Matrix can help enhance patient care and improve the efficiency of physician practices, as well as assist practices in achieving clinical targets in pay-for-performance contracts.

 

Matrix houses disease management and preventative care programs including Diabetes, Pap, Mammography, Asthma, and Chlamydia. Diabetes, Pap, and Mammography are the adult measures in Matrix. Asthma and Chlamydia are the pediatric measures. The patients loaded into Matrix have been identified as having Diabetes or Asthma, or as needing a Pap smear screening, Mammography exam, or Chlamydia exam.

 

The data in Matrix comes from claims data (HPHC, TAHP, SH, BCBS) and is refreshed on a monthly basis, with the exception of the BCBS data. Users have the ability to manually add patients with other health plans to the system.

 

Matrix was piloted in the summer of 2003 and was rolled out to the PCHI network thereafter. There was 100% adoption by June 2004. The majority of RSOs are using Matrix at the practice-level, although a few RSOs are using Matrix centrally.

 

Screen shot of Diabetes Summary screen: Displays non-compliant patients (i.e. patients who are in need of either an Eye exam, HbA1c test, LDL screening, or Microalbuminuria test). Users have the ability to print initial/reminder letters that can be sent to the patient, call lists, and chart inserts.

How to get access to Matrix:

Matrix is a clinical application that is accessible via PCHInet. All users need to be enrolled in PCHInet as clinical users before they can be given Matrix access.
